What is care management?

We work collaboratively with health care providers to identify individuals at risk for hospitalization and/or Emergency Department visits due to their challenging medical and/or psychosocial needs. Our Nurse Manager with her team, which includes Nurses and Allied Health Professionals, provide these services through professional assessments and promotion of quality and cost-effective interventions to ensure optimal client wellness and autonomy.

People who would benefit the most would be patients who have just been discharged from hospitals, families who are facing complicated medical situations or families who require caregiver training for the main caregiver of their loved one.

What does the nurse manager do during the care assessment?

The nurse manager meets the patient and family and does a detailed assessment across the different health aspects (physical, mental, social, etc). She identifies areas of need and/or desires and then develops a customised care plan to address those needs and desires. This plan will connect the patient to the rest of the team and various community resources and programs to help the patient enjoy an improved quality of life.

I have been informed by the hospital that my loved one is scheduled for discharge within the next 2 days. What should I do?

We will conduct an assessment and assess the needs of your loved one. Within a short span of 2 working days, we can arrange for consumables to be delivered to your home address and ensure that equipment like bed or commode are all prepped and ready for use in your home should your loved one requires it. Following that, on the day of discharge, we can arrange a medical escort to bring your loved one home and settled him / her down with you. We can also conduct trainings on how to care for your loved one at home and how to use the equipment as well.
